Academic Records

If you require a copy of your academic record from the Classic site, please complete the form and we will provide you with a PDF copy.

Colleges may also request a copy of the academic records of all their students.

Provide the name of the ICPKP Affiliated College you attended.
Provide the name of the ICPKP Faculty Member who ran the college you attended.
Provide the login name you used to access the Classic ICPKP site.
If you had a different email address recorded on your Classic site profile, please provide it.

Upload a photo or scan of your ICPKP certificate, diploma or other evidence of your ICPKP studies. Supported formats: JPG, PNG, HEIC, PDF.
Please provide any further details to help us confirm your identity (eg Maiden name), and any specific requests regarding your Classic website academic record.
